Studio Project Fall 2025 - Parent/Guardian Liability Waiver
Boulder Museum of Contemporary Art is pleased to have your child participating in our 2024-25 Studio Project Teen Program! This year's Studio Project Teen program is a collaboration between BMoCA and Firehouse Art Center.

Each Semester Teens Will:
-Explore various art making styles & mediums;
-Select guest artists to serve as artist mentors;
-Discuss social justice issues & create opportunities to impact their communities; and
-Share their artwork & ideas with others through art shows or gatherings.
Meeting Locations Most meetings will be held at BMoCA, 1750 13th St, Boulder. Occasional meetings will be held at Firehouse Art Center, 667 4th Ave, Longmont. If necessary, virtual meetings will be scheduled.

Fall 2024 meetings will be held Wednesdays from 5:30-7pm:

I hereby permit my child to participate in BMoCA’s 2024-25 Studio Project Teen Program. I understand that my child is wholly responsible for themself during meetings and field trips, and while visiting the galleries at BMoCA and Firehouse Art Center.

1.  I exempt Boulder Museum of Contemporary Art, its employees and authorized volunteers from all claims arising from my child’s participation in BMoCA’s programs and surrounding activities. 

2.  I understand that my child’s participation in the programs is voluntary, and that by participating, such participation potentially involves risks and obligations that are impossible to predict.  These may include, but are not limited to:  the risk of loss or damage to personal property, the risk of sickness, personal injury, or death while participating in the program. 

3.  Boulder Museum of Contemporary Art personnel are not authorized to administer medication of any sort. 

4.  I will provide Boulder Museum of Contemporary Art with an emergency phone number where I can be reached during BMoCA’s programs.

5.  I understand that I am solely responsible for arranging transportation of my child to and from BMoCA’s programs.

8.  I understand that the Boulder Museum of Contemporary Art does not purchase, or have any medical, dental or hospitalization insurance to cover injuries to or loss of life of students, or to indemnify parents or guardians for expenses in connection therewith, and that such insurance, if desired, must be purchased by the parent or guardian.
