This form is for Retailer / Installer Businesses to provide information about their business so that Michael & Alecia Sparks with AMSOIL / Lube-Direct can learn how to best help them. This questionnaire will only be used by Michael & Alecia Sparks. Your information will not be shared and will remain confidential.

The intent of this questionnaire is to better understand your business – how you choose your lubricant vendors and products, your target margins and any improvements you’d like to make or gaps you’d like to fill in your current lubricant offerings. Our goal is to determine if AMSOIL products would be a good fit and help improve the profitability of your business. In order to do so, we'd like to ask some questions. This will help us put together a proposal unique to your business. We will set up a follow-up meeting with you to discuss our proposal, which will include pricing.

What type of business do you own? Typically there are several types of business that retail/install AMSOIL Products. 
Retail Store e.g.: Auto Parts, Powersports, Hardware, Small Engine
Installer: Any lube shop/repair shop that installs products for customers' vehicles and equipment, e.g. Automotive, Diesel, Performance, Small Engine, Powersports. 
Combination Retail/Installer: Any business that sells products to walk in traffic as well as installs and/or  repairs vehicles and equipment.
