So, are you ready to get together and talk about your week? Are you prepared to use the help you have access to?

Think of this as a way to get focused on what's important and take a measure of where you're at.

Your answers on this form allow me to be as prepared and focused on your issues and progress as you are. Edit your answers up until the day before we meet by using the link to submit a new set of responses. (I just pay attention to the latest answers.)

2. What did you accomplish in the past week? *
What one or two activities you followed through to completion (ALL THE WAY TO "DONE") in the past seven days. NOT what you worked on, what you actually accomplished.
3. What obstacles did you encounter in taking action or completing goals in the past week? *
Identify perceived barriers. For those things you wanted to accomplish but DIDN'T. What got in your way? What do YOU think kept you from getting to "done" with those activities?
4. What's the next action that will have the greatest impact on achieving your goal? *
What one thing CAN you get done next week that will help you keep moving on your path? This is a task or project that you 1. CAN fully accomplish that 2. WILL also move you forward toward your goal.
5. State your assumptions about how you will accomplish that goal? *
A. What problems do you expect to encounter? B. What resources will you need? C. What do you assume will be easy/difficult? D. Who will help/block you (and why)? Make ALL your assumptions explicit. This is a checking tool that allows you to improve your goal setting in the future. The better you get at accurately stating your assumptions the more likely you'll accomplish your goals.
6. What question(s) do you have that the group or I can give you feedback on?
THIS is going to be our agenda for conversation. The clearer you are about what's troubling you and what you need, the easier it is to get good advice. A. What 'isn't working' or B. What 'something else' I am looking for. (Feel free to be verbose to start and sum your thoughts with a question sentence at the end.)
