Welcome to 56ers at Faith Lutheran Church! This form is your way to stay tuned into what events and programs are coming up! It is also a simple way for me, Laura Stowman, to get a hold of a parent in case of emergency. Thank you for filling this out!

What is 56ers?
56ers is an after-school ministry for ANY student in 5th and 6th grade. Students do not need to be members of Faith Lutheran to participate. At 56ers we begin with hang time, followed by snacks, Bible lesson, and wrap up with a game. 

Dates
56ers starts Wednesday, September 10, 2025, and ends Wednesday, May 20, 2026. The time is from after school until 5:15 p.m. There will not be 56ers on early release days, no school days, if school ends early due to weather, and October 29. 

Transportation
6th grade - a bus will be at the end of the bus line at the Middle School to drive over the 6th graders along with our Confirmation students after school. After getting off the bus, this group is welcome to walk over to Domino's, Benny's, DQ, or Below Zero before walking into the church. However students need to have permission from their parents AND tell Kristen and I which place they are going before walking with their friends. Once in the building they cannot leave.
5th grade - an adult will meet students at the large rock at the Southeast corner of the Park Elementary grounds to walk over to the church.

I give permission for my child to take part in all children activities that are a scheduled part of Faith Lutheran Church's Children's Program. I hereby agree to release Faith Lutheran Church, its sponsors, representatives, and agents from any legal liability for my child’s participation in these activities. I further agree to save, defend, and hold harmless Faith Lutheran Church from any claim of legal liability for stated activities. I understand that this permission granted will cover all activities from September, 2025 to September, 2026.

I hereby grant Faith Lutheran, its representatives and employees the right to transport the identified child/children to program-run activities and field trips. I understand that any vehicle in which my child/children will be transported will be operated by an adult over the age of 21. I understand that I will be notified of any off-site activity to which Faith Lutheran plans to take my children and that I retain the right to request that they not attend any off-site activity.

I understand that every effort will be made to protect and safeguard all youth at Faith Lutheran Church activities. Therefore, during the period from September, 2025 to September, 2026, I agree not to hold Faith Lutheran Church or its designated representatives liable for any illness or mishap from any cause whatsoever which may be sustained. I understand that medical or surgical emergency, after every reasonable effort has been made to contact me, the child’s physician, or the emergency contact person, I do hereby grant my permission to the physician(s) selected by the leader and/ or Faith Lutheran representatives to provide any treatment or procedure deemed necessary for my child.

I understand that any child blatantly disregarding the authority of Faith Lutheran church’s designated representatives are subject to being sent home at their parents’ expense with no refund of the activity fee. Likewise, any child who willfully destroys property will be held responsible and charged accordingly.

Photo Release

I hereby grant Faith Lutheran Church, its representatives and employees, the right to take and use photo, video, or audio files of the above identified child. I authorize Faith Lutheran, its assigns and transferees to copyright, use and publish the same in print and/or electronically.

I agree that Faith Lutheran may use such photographs of my child without their names and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and web content.
