Thank you for being involved in T.I.D.E. LLC! To commemorate and promote our activities, we would like to capture photos of participants. Please read the following consent form carefully and indicate your agreement by signing below.

Consent to Photograph and Use of Images

I, the undersigned, hereby grant permission to T.I.D.E. LLC, its representatives, and employees, to take photographs of me and to use these photographs in any and all of its publications, including website entries, social media platforms, and email communications, without payment or any other consideration.

I understand and agree that these materials will become the property of T.I.D.E. LLC and will not be returned.

I hereby irrevocably authorize T.I.D.E. LLC to edit, alter, copy, exhibit, publish, or distribute these photos for purposes of publicizing T.I.D.E. LLC's programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears.

Additionally, I waive any right to royalties or other compensation arising or related to the use of the photographs.

I hereby hold harmless and release and forever discharge T.I.D.E. LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Consent for Use of Names

I, the undersigned, hereby grant permission for the use of my name in connection with photographs, videos, or other digital images taken of me by T.I.D.E. LLC. This consent includes the use of my name in any media, including but not limited to print, digital, and online platforms, for promotional, educational, or informational purposes related to the activities of T.I.D.E. LLC.

I understand that my name may be used alongside images of me and may be included in captions, articles, or other content produced by or for T.I.D.E. LLC.

By signing this form, I acknowledge that I am consenting to the use of my name as specified above and waive any rights or claims to financial compensation or rights to the final product.
