We are looking for any feedback you may have on this proposed set of Foundational Public Health Services. Please submit comments and questions on the overall model or on specific pieces of it using the form below. Your comments will help us with the next iteration of this work.

Do the concepts or notions put forth in the Foundational Public Health Services model resonate with you? If yes, how so? If not, why not? Please be as specific as possible.
Acknowledging that the majority of a health department's work will fall "above the line", are there areas or services you consider foundational that are missing from this model? If so, please explain why.
Please share specific feedback on the Foundational Capabilities (FCs). This might include why you agree they are “foundational,” or why you do not think they are. You can also comment on the scope of activities we have outlined in the V1 document. Please be as specific as possible.
FCs are cross-cutting skills that need to be present in state and local health departments everywhere for the health system to work anywhere. They are necessary to support the foundational areas, and other programs and activities, key to protecting the community’s health and achieving equitable health outcomes. These skills include organizational competencies such as leadership, governance, quality management, and health equity; all hazards preparedness and emergency response; assessment; communications; policy development and support, and community partnership development.
Please share, in as much detail as possible, feedback specific to Foundational Areas (FAs). This might include why you agree they are "foundational," or why you do not think they are. You can also comment on the scope of activities outlined in the V1 document.
FAs are those substantive areas of expertise or program-specific activities in all state and local health departments also essential to protect the community’s health. Examples include communicable disease control; chronic disease and injury prevention; maternal, child, and family health; access to and linkage with clinical care, and environmental public health inspections and monitoring.
