The purpose of this form is to identify some characteristics of your child to assist with classroom placement for next school year. Its purpose is not to identify a specific teacher. Please note that specific names of teachers cannot be accepted. Only descriptors will be considered for classroom placement. In the space provided below, please check any information (descriptor words) which describes your child. Include any additional teaching styles, or other unique needs that you would want us to consider in classroom placement for the next school year.

I understand that this request is just one of many variables that may be considered in the grouping/assigning of students for instructional purposes. Other factors include work habits, ability, special individual and building needs, and the balance of boys and girls. Fluctuations in enrollment may create the need to increase or decrease class size. For these reasons, I understand that the total educational program must have the highest priority, and the final determination of classroom placement is the responsibility of the building principal.
