Schedule Change Requests, Spring 2019
ALL requests MUST be made inside this form. Phone call or email requests will NOT be honored. To be considered, student must first select, from a drop down list, the category that the request falls into. Student MUST then write in the name of the course they wish to drop, MUST write in the name of the course they wish to take in its place, and MUST provide a reason. If any of these pieces are missing, request will not be considered. You MUST sign up for your grade level remind group in order to be notified of your schedule change request. If this presents a problem for you, please see your counselor AS SOON AS POSSIBLE. This form will close at 8am on Friday, December 7. PARENT/GUARDIAN IS EXPECTED TO BE PART OF THIS PROCESS. MAKE SURE THEY KNOW OF THE REQUEST YOU ARE MAKING, AND HAVE THEM PROVIDE AN ELECTRONIC SIGNATURE BELOW!

* Missing a required core course on schedule (English, Math, Science, Social Studies)
* Scheduled for a course that you have already earned credit in, and is not repeatable: Horticulture I, English II, etc.
* Scheduled for a course in which you do not have the required pre-requisite: Scheduled for Horticulture II, never had Horticulture I, scheduled for Math III, never took/passed Math II
* SENIOR only who can provide documentation from a college that they need a specific course such as an additional World Language or an Art credit (rarely is anything else required by a college)
* JUNIOR OR SENIOR that would like to look at adding an HCC course for college credit.

Changes that will not be considered:
* You requested a course, either as a first choice or as an alternate
* You requested, and qualify for an Honors/AP/HCC class, but you have changed your mind, or taken on a job, or an additional extracurricular since you requested
* You play a sport & would prefer your 4th block class to be something other than what it is
* Request based on teacher preferences
