Day of ADA 30th Anniversary Celebration Registration Form
The ADA Virtual Anniversary Celebration will last one hour and include six different activities in that time period. Attendees who participate in at least 4 of the 6 activities will get a gift card to Chocolate Moose AND the opportunity to receive a $50 Visa gift card (five will be given out).
We need for attendees to provide the  information below as it is necessary for the safety of the Zoom activity. Personal information will not be shared or be put on any list. You will receive more information and the link to the Zoom 24 hours before the event on Thursday, July 23rd, 11:00 a.m. - 12:00 p.m.
