2020-2021 Concurrent Enrollment Contract
Congratulations on your decision to take an online college course through Arizona State University for the 2020-2021 school year. This is a great opportunity for you to receive high school and college credit for the same class, have an authentic college experience, and get ahead in your college career.

All students must sign the below acknowledgment form at least one week prior to the start of academic courses. Please review the below information carefully and reach out to your Learning Success Coach with any questions.

Where applicable, students are responsible for all incurred charges via their myASU account. This includes the purchase of books via the ASU Bookstore, Suncard, or parking passes. These payments must be reconciled in a timely fashion to avoid late fees. Students with past due ASU accounts will not be eligible for course registration until balances are paid in full.

Students with past-due accounts may be ineligible for participation at ASUPD sponsored events such as prom, graduation, etc.

1. I will abide by the same academic and behavioral policies required of college students outlined in the ABOR Student Code of Conduct. Failure to do so could result in college disciplinary action. ASU Prep Digital will not be held responsible for any academic or behavioral conduct violations on my part. I understand that I will be solely responsible for handling any disciplinary action commuted to me by ASU. *
2. I agree to sign the ASUPD FERPA form that allows ASU to send my final grade to ASU Prep Digital. *
3. I agree to view the videos in the concurrent course contract prior to starting my course(s). I agree to reach out to my LSC with any questions regarding this material. *
4. I understand that I am required to send monthly progress updates (via screenshots) including point tallies and grades to my LSC. *
5. I understand that if I receive a D or F in a concurrent course, I am ineligible to take concurrent courses in the following semester. If I receive a C in a concurrent course, I am limited to one concurrent course in the following semester. *
6. I understand that it is my responsibility to stay on or ahead of pace in all classes and maintain passing grades at all times. Failure to do so will prevent me from taking future concurrent courses at ASU. *
7. I understand that each course syllabi contains important information regarding grading policies, class structure, due dates, and how to communicate with my instructor. I agree to thoroughly read through the syllabus and to communicate any concerns to my instructor and LSC as needed. *
8. I understand that if I have an IEP or 504 plan in place that it is my responsibility to contact the ASU Disability Resource Center on the ASU campus (https://eoss.asu.edu/drc) to request appropriate accommodations. *
9. I understand that ASU courses are university-level courses and content is not modified in any way for high school students. Courses may contain mature or adult themes. *
10. I understand that failing a college course will be posted to both my high school and college transcript and will affect my incoming college GPA. *
11. If I am taking a college course to meet my high school graduation requirements, I understand that failing the course will jeopardize my ability to graduate on time. I will be off-track for graduation and my GPA for high school and college will be affected. *
12. If I choose to drop my concurrent course, I agree to notify my LSC immediately to discuss the impact this may have on meeting critical graduation requirements. *
13. Every semester-long class at ASU (3-5 credits) will be transferred back to my ASU Prep Digital (high school) transcript as 0.5 credit. If I am taking a 2-credit course, I understand that I may need to enroll in additional high school courses to ensure I am on track for graduation. *
14. I understand that I will need to complete two semesters of a college course in order to earn 1.0 credit that meets applicable graduation requirements. *
15. I understand that it is my responsibility to verify my enrollments in myASU account. ASU Prep Digital will not be held liable for any registration errors (incorrect section, incorrect course, etc.) that may occur. *
16. I understand that ASU Prep Digital will invoice me for payment and fees through the parent portal and is due prior to the start of ASU courses. I understand that failure to make payment will result in the administrative withdrawal of my concurrent course(s). If I have a previous balance, my outstanding fees and tuition must also be paid in full prior to enrollment. *
17. I understand that college courses often require textbooks to be purchased. I understand that ASU Prep Digital will not provide textbooks (Textbooks may be bought in ASU’s campus bookstore, purchased online or rented through a textbook rental agency at my expense.) *
18. I understand that I am responsible for any books, materials, or charges made to my ASU student account. This includes but is not limited to: purchase of books via the ASU Bookstore, ASU Suncard, parking passes. These payments must be reconciled in a timely fashion to avoid late fees. I understand that if I have a past due ASU account, I will be ineligible for future ASU course registration and barred from attendance at in-person events such as ASUPD Graduation until balances are paid in full. *
19. I understand that if I have a tuition waiver from ASUPD and drop the course, receive an F, or stop working after the full-refund add/drop period, I will be responsible for repaying the tuition cost to ASUPD. *
20. ASU’s policy on grade forgiveness allows students to retake up to 12 credits in their lifetime at ASU. Should I wish to retake a course for grade forgiveness, I will be responsible for tuition and all costs incurred. *
