Carl Cozier After School Enrichment Scholarship

Dear Parent/Guardian,                                                                                                                    

Your Carl Cozier PTA strives to provide a variety of enrichment opportunities for every student at Carl Cozier. Many of these activities have fees associated with them. We are sensitive to the fact that money is tight for many families. And, in an effort to formalize the process in which families can request financial assistance from the Carl Cozier PTA, we have established the following set of guidelines for the 2023-2024 school year. 

 2023-2024 Guidelines for applying for Financial Assistance

  1. Financial assistance will be awarded for one Carl Cozier PTA sponsored activity per trimester. Financial assistance will not be awarded for multiple activities. 

  2. All information requested on the form below must be completed. Incomplete applications will be returned unprocessed. 

  3. Due to the increasing number of financial assistance request and limited funding, completed applications will be considered on a first come, first serve basis. The Carl Cozier PTA Executive Committee will review all applications in the order in which that are received. The Carl Cozier PTA will notify families of decisions as soon as possible.
