2026 KCA Hall of Fame Nomination & Application Form
This application needs to be completed.  Some of the information needs to come from the nominator and some from the nominee.  The nominator can do this as a surprise about the nominee but will need to get the information needed and include on this form or the nominator can share with the nominee that he/she has been nominated and have he/she complete their own information. Application deadline is 4:00 pm on Wednesday, January 21, 2026.  Completed Application, two letters of reccomendation, and a photo are all due at this time.  See bottom of application for more details.
