Welcome to the Extended Learning schedule change form! You may use this form to make a change to your overall schedule, or to all a single morning or afternoon.
Additions of AM care will be $7.00 and require a 48 hour notice and an email to the SITE COORDINATOR.
Additional days will be $38.25 per day and require a 48 hour notice. You will receive a confirmation email when your request is approved.
Please note there will be no 'switching days'. Any extra days needed will need to be added at $38.25.

All continuous schedule changes will result in a $15 change of schedule fee.
All schedule changes must be in effect for at least a month, or days will be added on an individual basis.

PLEASE NOTE ALL SCHEDULE CHANGES REQUIRE 48 HOUR ADVANCE NOTICE. Please always email your site coordinator and your student's teacher with schedule changes.
