IMPORTANT: An electronic copy of this form will be sent ONLY to the English Department Chair through Google Docs. Google Docs data is stored off the VCCCD servers, and no one else will have access to individual faculty information. Only aggregate data will be reported for each course.

Once you have filled in the form below and before you press submit, PLEASE PRINT FOR YOUR RECORDS and for upcoming discussions with other faculty teaching this course to complete the "Course Level SLO Assessment Summary Sheet."
