General Instructions
The Rhode Island Department of Health (RIDOH and its new Community Heath Worker (CHW) Program need some basic information about you and your organization. We are working together to TRAIN, DEPLOY, and ENGAGE CHWs to build healthy and resilient communities under the federal Community Health Workers for COVID Response and Resilient Communities (CDC-RFA-DP21-2109) grant award to Rhode Island .

This form will collect your contact information and other important details about your work with us. This helps us to better communicate, respond, and support your organization in an efficient and timely manner.

CHW Strategy Team Participation: 

Partner organizations funded through the CHW grant are encouraged to attend CHW Strategy Team monthly meetings. Individual CHWs, employers, and other partners are strongly encouraged and welcomed as well!

Virtual (online) meetings are held on the first Wednesday of the month from 3:30 p.m. to 4:30 p.m. No meetings are scheduled in July and December to accommodate summer and winter holiday breaks.

At most or all meetings, Strategy Team members will move into one of the four workgroup breakout rooms to address:

1. Workforce development
2. Advocacy
3. Sustainability
4. Interagency collaboration

4. Envision Equity

Envision Equity serves as the national Community Health Worker (CHW) Training and Technical Assistance Center in support of the CDC CCR grant. Resources are evidence-based and created by CHWs. Everyone is welcome to attend these free trainings!
