Certified Staff Student Contact Log
This form is to be used to document all time spent toward your Student Contact Stipend. It must be completed in its entirety and submitted electronically in order for you to receive your stipend.
Please keep in mind that you will be paid for hours only if they comply with the following:
*Tutoring - must be equal to at least 1/2 of total hours claimed; must have been included
on your approved Student Contact Stipend Plan in the fall
*Direct Student Contact - must have been included on your approved Student Contact Stipend
Plan in the fall
*"Other" Activities - may not be equal to more than 1/4 of total hours claimed; must have been
included on your approved Student Contact Stipend Plan in the fall
*All activities must take place prior to 7:45 a.m. and after 3:30 p.m.
If there are going to be any variations to the above, they must be pre-approved by the administrator.
