I have looked over the design and checked the spelling if applicable and give my full consent to the applications of my tattoo.

I acknowledge that I an not pregnant and have discloses all medical conditions.

I do not have HIV, Hepatitis or and other blood infection that might infect others.

I am 18 years of age or older or between the ages of 16 and 17 with written consent from a parent and a parent available to be present.

I give permission for any images of my tattoo to be used for media purposes.

I acknowledge the variations in color and design may exist.

I acknowledge that I was instructed on proper care of my tattoo, and recognize the absolute necessity of following those instructions.

I understand there are no refunds.

I agree to release and forever hold harmless nsink210 and its artists and shareholders from any and all claims damages or legal actions arising from or connected in any way with my tattoo.

I acknowledge that there is a chance I might feel lightheaded or dizzy during or after being tattooed. I agree to immediately notify my artist in the event I feel lightheaded or dizzy and or faint or if I have fainted in the past during any procedure.

I agree to follow all instructions concerning the care of my tattoo and any touchups due to my own negligence will be done at my own expense.

I have been fully informed of the risks of tattooing, including but not limited to: infections, allergic reactions to pigment or other materials used,

I have been informed of the potential risks associated with getting a tattoo and I still wish to proceed with the application and I assume any and all risk that may arise.

I confirm that all information I have provided is correct to the best of my knowledge.
