Birthday Club Sign-Up Sheet
Please use this form to sign your child up for the Birthday Club.  The parent MUST be a subscriber to enter the child.  If you have multiple children, you must sign each one up separately.  You CAN use the same parent email for multiple children.  Our system does NOT allow you to be sent more than one newsletter weekly, so do not worry about using your email more than once, as you can only be subscribed once.  This Birthday Club is open to kids aged 16 and under who live in Framingham, Natick, Sudbury, Ashland, Southborough, Weston, Wayland and Dover.
