This form is to be used by a parent or guardian to report an absence from school at DACC. Please answer each question and be sure to click submit at the bottom.

**Please remember, if your students home school is closed they are still required to attend DACC-transportation is still provided by their home school. If your student is not present, they will be considered absent for the day**
** If you have already completed a pre-arranged absence form, you do not have to complete this form**
**Please note that if you enter this after 9:15am (AM Labs) or 12:50pm (PM Labs), you will still receive the call/text/email reporting the absence of your student**
