I certify that I have rented/borrowed an instrument from the school listed above for the 20-21 school year. It is agreed that if the instrument is damaged due to any cause, with the exception of normal wear and tear from careful use, I will pay for the amount of such damages and the cost of replacement of the instrument if the instrument is lost or is not repairable. Only district approved music technicians will be allowed to repair damaged instruments. I further agree that no person, other than me, will be allowed to handle or use the instrument without the consent of the Director. I agree to surrender the above instrument and accessories immediately upon the request of the Director or other school officials. A copy of the original rental contract will be available in your child's instrument case or folder/packet when the instrument is picked up from the school.
