Nominations for the 2025 IEEE Canada Major Achievement Awards
This is a one step nomination process.  Ensure you are aware of the purpose for each award and the qualifications for each award.  Failure to provide sufficient evidence that can be verified by the evaluators will adversely affect the outcome decision.  Information to aid your nomination for any of the seven (7) IEEE Canada Achievement Awards is available here:  https://drive.google.com/file/d/1GZVqJuz1YNUVI-MaFhWK4Rl3FsDz10TG/view?usp=sharing

NOTE:   A separate nomination form is available for the IEEE Canada Major Service Awards.

Anyone can serve as a nominator or endorser.  A nominee or endorser may support any number of nominees.

Nomination or endorsement by any member of the IEEE Canada Awards & Recognition Committee or voting member of the IEEE Canada Steering Committee, Executive Committee or Board of Directors is not permitted.  Self-nomination is not permitted.

The FIRM DEADLINE for submission is the end of Sunday, April 06, 2025.  Any query may be directed to arc@ieee.ca no later than April 02nd (four days prior to the deadline).

3.0  Upload Endorsements #1, #2, #3.  Nominator logs into this form using their GMAIL email address to upload the endorsements.  A minimum of two and a maximum of three endorsements are needed and can be combined into a single PDF file up to 1MB or kept as individual files of up to 1MB each for upload.  Add nominee's name to the end of the file name. *
