Application Form for 2017 National Churros Eating Championship
Terms and Conditions:

1. Spend any amount in any Street Churros stores in Malaysia. (Please write down your receipt number and date of purchase to complete the application form)
2.   Each application requires three (3) members to form a team to participate.
3. Three (3) teams will be selected for each preliminary round of competition in each Street Churros store as stated below.
4. Selection of participants for this competition is solely at the discretion of the organiser and no objections will be entertained.
5. Applications without contact numbers and email addresses will not be considered as  successful participants shall be notified by phone or email only.
6. The winner from each Preliminary Round in each Street Churros store shall qualify for Grand Finals and stand a chance to win FREE Churros for 1 year.



Locations of Preliminary Rounds:
1) Street Churros @ Berjaya Times Square        
- 3pm, 15th of July 2017
(Submit application by 9th of July 2017)

2) Street Churros @ 1 Utama        
 - 3pm, 22nd July 2017
(Submit application by 16th of July 2017)

3) Street Churros @ Empire Shopping Gallery, Subang                                                                        
 -3pm, 29th July 2017
(Submit application by 23rd of July 2017)

4) Street Churros @ IOI City Mall, Putrajaya      
- 3pm, 5th August 2017
(Submit application by 30th of July 2017)

5) Street Churros @ MyTOWN Shopping Centre, Cheras                                                                                  
 - 3pm, 12th August 2017
(Submit application by 6th of August 2017)

6) Street Churros @ Sky Avenue, Genting Highlands      
 - 3pm, 19th August 2017
(Submit application by 13th of August 2017)


GRAND FINALS:
LOCATION: STREET CHURROS @ BERJAYA TIMES SQUARE
DATE : 16TH SEPTEMBER 2017
TIME : 3PM
