Cost is 200 USD at time of registration per lab representative.

5 USD per certificate (1–1000)
4 USD per certificate (1000–5000)
3 USD per certificate (5000–10000)
2 USD more than 10000

Laboratory principal investigator
This person will be responsible for certifying results at this laboratory. This person should be a credentialed medical professional.
