Welcome to Cat’s Twilight adoption program. We request the following information so that we can assist you in the selection of a new cat. This form and a consultation with a Cat’s Twilight representative are designed to help you find the cat most compatible with your lifestyle.  

To be considered as an adopter, you must:
• Be 21 years of age or older
• Have identification showing your present address
• Have the knowledge and consent of your landlord
• Be able and willing to spend the time and money necessary to provide medical treatment and proper care of the cat

Completion of this application does not guarantee adoption of a Cat’s Twilight cat. Please complete all questions. Answer truthfully, not what you think we would want to hear.  Any misrepresentation on this application will be grounds for denial.  Thank you!

The cat that you are applying for will be part of your family for the next 15 to 20 years.  Each cat has its own distinct personality and preferences, some of which you can help shape, others which you cannot change.  Pets make considerable demands on your time and resources.  Cat owners need to spend time cleaning litter boxes, socializing, grooming, feeding and playing with their cat every day.  Cats that are shy or have not had much human contact need even more time and socialization.  Are you ready to accept the personal and financial responsibility of owning a cat? *
