Event: Crawford Hoops Basketball Camp
WHAT:  Crawford Hoops Camp will teach your child the basic fundamentals of basketball and an opportunity to learn and improve their basketball skills.  Ball handing, dribbling, passing and shooting will be practiced daily.  Offensive and defensive skills also be incorporated.  We are here to HAVE FUN while introducing and facilitating the game of basketball.  A concession stand will be provided with drinks and snacks.   (There will be a "bank" that you can deposit money in and leave for your child)

WHEN:          June 1,2,3 Monday, Tuesday, Wednesday 

TIME:            Grades 2-5 Monday/Tuesday 9:00-12:00 (WEDNESDAY ONLY 9:00-11:00)
                      Grades 6-9 Monday/Tuesday 12:30-3:30 (WEDNESDAY ONLY 12:00-2:00)

PLACE:         Crawford High School Gym/Elementary Gym(We will bus grades 2/3 to Elementary Gym each day.                         *You will drop off and pick up each day at HS.

We, the parent(s) or guardians(s) of the above student(signee), hereby grant permission to participate in Crawford Hoops Camp.  We acknowledge the fact that they are physically able to participate in camp activities and hereby release the camp and its employee(s) from all claims of injuries or illness, which may be sustained by this participant during camp.  We authorize the camp director or his designee to select hospital facilities  and/or physician to treat the above named camper on any emergency basis in the event that such treatment becomes necessary while attending the camp.
