One nominee from each undergraduate institution is eligible for consideration for the Kaplan Undergraduate Achievement Award. A nominee should be a junior student enrolled at your institution in the 2013-14 academic year who is planning to pursue a career as a health professional. The student selected to receive the award will receive a $500 cash award along with an MCAT, a DAT, or a GRE Kaplan preparation course as appropriate for their chosen field. In addition to completing this form, the TAAHP undergraduate pre-health adviser must email:
(1) a copy of the nominee's unofficial undergraduate transcript (2) a letter of support detailing the reasons the adviser believes the student is a good candidate for the award (3) a one page personal statement written by the student explaining their personal and academic experiences that motivate them to pursue their professional goals.
All nominated students will be recognized at the annual TAAHP meeting in February. Please send your documents to support your nomination to Mary Wells at firstname.lastname@example.org.
