Fall 2026 Field Hockey Coordination Form

Thank you for your interest in Greenhills youth field hockey. This form helps us coordinate Greenhills teams with Ann Arbor Rec & Ed for the fall season. Please complete this form once your child has already registered with Ann Arbor Rec & Ed field hockey.

Team availability will depend on the number of registered players at each grade level. To help form complete Greenhills teams, some students may have the opportunity to play one grade above their current grade level.

Field hockey is open to students in 2nd grade through 6th grade. If you student is entering second grade, they will need to play in the 3rd/4th grade league.

Please note: This form does not register your child for field hockey. You must also complete the Ann Arbor Rec & Ed Registration Form.
