2024-25 JV Tennis Opt Out Non-Tryout Form
If a female athlete has a UTR above 1.5, and a male athlete has a UTR above a 2.5, he or she may choose to opt out of Varsity Tennis tryouts and be placed in JV Tennis. This form is not required if you would like to try out for varsity.

Pre-participation physical  
HPISD and the HP Sports Medicine Team require that students in 9-12th grade participating in UIL-sponsored athletic activities update their physical every year. The physical must be completed using the UIL Pre-Participation Physical form and the exam may be completed no earlier than May 1, 2024. Student-athletes will not be allowed to participate in tryouts, begin practice or competition without having this physical on file with the HP Sports Medicine Department. The "date of exam" field  must be dated May 1st, 2024 or later. Parent and student signatures are required as well.

Athletic Participation Forms
Highland Park ISD Required Forms for Athletic Participation require all online forms to be completed, signed and submitted electronically on or after May 1, 2024 for grades 9-12th. Paper copies will not be accepted.
You will need to complete this process separately for each individual student, but only once per school year.
