Future Vote Initiative: Teacher/Administrator Authorization Form
The Future Vote Initiative (FVI) is a year-round program for 6th-12th grade students to learn about elections and civic engagement.

A 1-hour training class is mandatory in order for your student to work at a polling place on Election Day.

Both parent/guardian and the student are required to attend the entire class.

As teacher/administrator, you are expected to apply credit or points to your student's portfolio when they work on Election Day and participate in other FVI activities throughout the year. If you are eligible, you are also expected to register to vote or update your voter registration.

As teacher/administrator, you must authorize the student to participate. Please provide the following information so your student can join the program. All the information you submit is used solely by the election office.
