The goal of this questionnaire is for me (Belle Amelie Artistry) to be able to fully understand what you’re wanting to accomplish for your personalized Photoshoot! Please read through this and get it back to me asap so we can start planning your personalized shoot!
I provide personalized/custom photo shoots that include family photos, couples photos, babies, maternity, portraits, business, boudoir, only fans work with posing coaching from a professional model (myself) and offer hair and makeup services as well! All photos will be sent digitally.

I understand that if client is more than 15 minutes late to Photoshoot session a fee of $25 will be assessed NO exceptions *
I understand that a NON refundable deposit of HALF the total due amount is due ASAP to hold your booking date *

I understand it will take about a week (7 days) to get edits back. If I need these photos rushed I will make that known ahead of time AND will plan my shoot date accordingly. In the event I need the photos rushed there will be a $35 fee due. *
