Baptism Initial Intact Sheet
Welcome to Saint Valentine's Church! We are overjoyed with the opportunity to welcome your child into not only St. Valentine's Church but also the Sacrament of Baptism which is God's most beautiful and magnificent gift.
Kindly fill out this form and submit so that we can begin the process of scheduling your Baptism. Once this form is submitted we ask that you and your chosen Godparents work to obtain the required letters and documentation necessary to stand as "Godparents'.

Godparent Requirements:
If at any time you have any concerns or questions feel free to contact our Baptism coordinator (Mrs. Marta Cirilli-Kuebler at 201-725-5621).
- Number: Only one Godparent is required, but if there are two, one must be male and one female.
- Requirements: Must be at least 16 years old (sometimes 14+ is accepted), fully initiated (baptized, confirmed, and received First Holy Communion).
- Practicing Catholic: They must be a practicing Catholic in good standing, not forbidden to act as a sponsor.
- Not Parents: They cannot be the parents of the child.
- Christian Witness: A baptized non-Catholic can act as a "Christian Witness," but at least one godparent must be a Catholic.
