A list of medications (Formulary) appropriate for the patients and as per the scope of the organisation‘s clinical services is developed collaboratively by the multidisciplinary committee. The list is reviewed and updated collaboratively by the multi disciplinary committee at least annually.

There is a defined process for acquisition of these medications* - (Vendor selection & evaluation)

Audit of medication orders/prescription is carried out to check for safe and rational prescription of medications (Appropriateness of drug dose frequency and route of administration, Presence of therapeutic duplication, Possibility of drug interaction and measures taken, Possibility of food drug interaction).

Documented procedure exists to capture near miss, medication error and adverse drug event.*
