Make Up Class Request Form
To request a make up class, please submit this form. You MUST select specific same level classes that work with your schedule and therefore may need to view our Class Schedule on our website prior to submitting. Please note that we may not be able to accommodate your request if we do not have space in the requested class. Our policy is no more than two make up classes per session. Make up classes are not guaranteed.
