This request form is to identify people impacted by food insecurity. We are a small group of donors, but will continue this initiative as often as we can. Please answer the questions below so we can identify the best way to support you. The goal is to have this project funded as soon as possible within 7-10 days. Number of recipients and size of gift card will depend on donations received in that time frame.
