The purpose of this form is to give people an opportunity to anonymously report concerns about the Chemistry Department, specifically department climate. Vice Chair for DEI, Amanda Holton, is the only faculty member reviewing this feedback and it will be held in strictest confidence. Feedback will be used to try to enact programs and create systems to foster an inclusive culture and provide a supportive environment for all department members. While examples are extremely helpful to understand types of changes that are needed, please be assured specific examples will not be relayed to anyone.
