Kindly fill in this form if you are born as of the year 2000 (ages 12-24) and interested in receiving the HPV vaccine. The staff from Primary HealthCare and HPDP will be on site on the day to offer this vaccine.
The HPV vaccine is FREE to ANYONE.

There are over 100 types of human papillomavirus (HPV). Gardasil 9 ® Vaccine covers 9 types of HPV and is over 90% effective in preventing certain types of cancer later in life and genital warts.
Note that the full course of vaccination in people aged 15 and over necessitates 3 doses at 0, 2 and 6 months. Thus, you will need to receive two more doses following this vaccination in order for it to be fully effective, for which we will organise future vaccination drives.
