Brisbane Water Oyster Festival 2019 Stallholder Application Form

Hello and thank you for applying to be a stallholder at this years Oyster Festival!

Please ensure you complete all questions in this application form.

All stallholders need to have Public Liability Insurance (at least $20 million.) We cannot accept your application without this. Please forward a copy after you submit this form to Emily admin@emptysuitcase.com.au

If you are a food vendor, we will also need your FSS certificate. We will send you details of the Temporary Food Permit (TFP) for this event once your application has been approved.

If you are an alcohol vendor you will need to forward copies of your RSA certificates.

• A copy of your current Public Liability Insurance Policy (All Stallholders)
• FSS and TFP details (Food Vendors)
• RSA information (Alcohol Vendor)

Once your application is successful, we will send you a confirmation email with stallholder payment details.
