Real World Testing Survey
All questions below are confidential and your responses will be recorded and used to comply with our Real World Testing certification with ONC HealthIT. We will keep a backup copy of your completed form in case our ONC-ACB requests that we present any documentation to back up our certification, but otherwise, the data provided will be translated to a report, keeping your name and any relevant personal information private. 

Keep in mind, these reports will be submitted anually and the veracity of the surveys may be verified through database audits; the data obtained will only be used to provide statistics regarding use of the certified modules. You may request removal from participating in these annual reports at any time by contacting our Support Team.

Please keep in mind that comments made within this form do not replace or complement any tickets you may or may not have open with us.
