List the date and time(s) as well as location(s) that the alleged offense took place.

List the specific part or parts of the SUUSA Constitution, bylaws, governing documents, policy, club constitution, signed contract, or any other law, custom, or standard of treatment allegedly violated, intentionally ignored, or deviated from in the official capacity of the alleged offender.

Please list details concerning how such alleged violation occurred.

Please list any other relevant comments or concerns to be sent to the Judicial Council when this form is submitted.
