AWARDS:
» Honoring Trailblazers (PAST) — Over 25 years of experience / service to the community
Recognize outstanding organizations and community leaders whose lives and stories exemplify courage and perseverance, whose commitment, despite obstacles, made a positive impact in the community.
» Honoring Community Champions (PRESENT) — At least 10 years experience / service to the community
Recognize outstanding organizations, individuals, leaders and advocates who work hard to improve the quality of life of members of the community and strive selflessly toward a goal that benefits many.
» Honoring Emerging Leaders (FUTURE) — Under 10 years experience / service to the community
Recognize successful, next-generation leaders who exemplify leadership, innovative thinking, vision and distinguished service to their organization or community.
NOMINATION CRITERIA:
1. Anyone who uplifts the profile of the Filipino community in the SF Bay Area is qualified.
2. Nominees must live or work in San Francisco.
3. The nominee is in good standing and of good moral character
4. Award recipients must be available to attend the Filipino American History Month Celebration to receive their award.
NOMINATION RULES AND REGULATIONS: 1. Nominators are encouraged to ask for their Nominee's permission and advise them of the event date.
2. All applications and their content — including any non-copyrighted photos or videos submitted — become the property of the Filipino American History Month (FAHM) Planning Committee.
3. Nominators and Nominees grant FAHM Planning Committee the use of their names and likenesses for publicity without compensation.
4. Nomination deadline is on Friday, September 28 at 12 midnight.
5. Award recipients will be notified on the week October 9.
