I hereby understand that a deposit of $20 will need to be paid prior before starting the Adult and Pediatric First Aid, CPR and AED Blended Learning and or re certification class. The $20 will be used to hold your spot in a class and be put towards payment at the end of the hands on in person session.
The $20 deposit will not be refunded to you if No call, No Show is implemented. Signing and submitting this form Indicates that you've thoroughly read and understand the Agreement form with True Blessings CPR Instructors.
