REPORT OF SCOUT SERVICE PERFORMED
It is important to report service you have done as a Boy Scout. Our council and district keep track of service performed by all Scouts and Scouting adults. Boys will also need to document their service in order to receive advancement credit for it. This form is used to report service you have performed outside of the Troop 202 organized troop-wide service projects.

You DO need to complete this form for:
- Service projects you performed at your church, school, or other location.
- Service projects completed by your patrol.

You do NOT need to complete this form for:
- Fish Fry dinners
- SetonFest parking or other service
- Service projects conducted by the whole troop during an outing
- Eagle candidate service projects (It's the Eagle candidate's obligation to report this.)

Usually, it is best to complete one form per Scout, but if the work description and hours are the same for a group of Scouts or can be clearly listed and understood, the hours could be entered in a single form.

Who did the service?  Unless the hours and work description are the same for all, it's best to complete a separate form for each Scout or adult.

On what date was the service performed?  (e.g., 03/15/21)
TIME *
At what times did the project begin and end?  (e.g., 2:00PM - 5:15PM)
HOURS *
How many hours did the work last?  (e.g., 3 hours 15 minutes)
BENEFICIARY *
For whom was the work performed?  Name the organization or individual that was the beneficiary of your service.  (e.g., Carmel Clay Parks, St. Elizabeth Seton Church, etc.)
LOCATION *
Where was the service performed?  (e.g., Cool Creek Park, Carmel Clay Public Library, Carmel Care Senior Community)  This may be the same as the beneficiary, but sometimes it is different.
ACTIVITY *
Describe what the activity was.  (e.g., planted trees, shelved books, or read to senior citizens)
CONSERVATION-RELATED? *
CONSERVATION CATEGORY?
If your project is conservation-related, indicate which of the following categories it falls in. The description of work provided above must support your classification. If only some of your hours are conservation-related, enter them in a separate report.
