2024 DC MS Summer Reading Book Form
Complete this form for each book you read this summer for Delaware County Christian Middle School. 
You MUST complete a form for the required reading books, as well as all other books you read.
Include your school email address or a personal email address if you are new to DC.
With your parent's permission, you may use a parent's email address, if you do not have one of your own. 

Summer Reading begins the 1st day of summer break - June 7, 2024. No forms will be accepted before this date.

All forms should be submitted by the first day of school,  August 27, 2024.
