Directions: The Google Form will close when the tournament is FULL. If you are a manager for multiple teams, please submit one form for each team. Your team will be fully registered once your team manager completes this form, completes payment, AND receives a confirmation email from Madison United.

The early bird price is $485. The early bird window closes on Thursday, January 8th at 11:59pm.

The regular tournament price  of $535 will be applied for any team registering on or after January 9th.

Champion teams in each division will  receive free registration for next year's tournament & the coveted Championship T-Shirt for each player on the roster.

Men's Open: We are hoping for 24 teams. If you are the 25th team to sign up, you will be on the waitlist.

Women's Open: We are anticipating 16 teams.

Men's Over 30: We are anticipating 16 teams.

Women's Over 30: We are anticipating 8 teams.

Men's Over 40: We are anticipating 8 teams.

Men's Over 50: We are anticipating 8 teams.
