Graduate Students Wellness Survey
This survey is designed as part of an effort to identify, understand, and tackle the challenges facing graduate students.  Your answers are thus very critical to us and will be used to guide the discussion during the <follow-up> seminar (or town hall) on <insert-date>.

We are aware that some of the questions are personal and sensitive. We will keep your individual responses private and anonymous, and only speak in <seminar> about the overall trends. We will endeavour to be careful about any responses that might lead to your identification; if there are any specific issues that you think might involuntarily identify you despite our best efforts, please either let us know in the comments (or refrain from inputting them).

Further, if there are any questions that you feel do not apply to you or about which you have no opinion, you can choose to not answer, as most questions in this survey are optional. The more input you can provide though, the better! Thank you!
