Waiver of Liability/Indemnification Clause & Photo Release

Participation in a martial arts involves physical contact and there is a risk that serious injury can occur. While understanding this risk, the undersigned student and/or parent/guardian assumes the risk of injury and assumes responsibility for his/her own safety. In consideration of my participation in the martial arts, the undersigned student/parent/guardian does for themself, their heirs, their executors, and administrators waive, release, discharge, and hold harmless Zodiac Combat Sports LLC, dba Nine Tigers Kung Fu, its owners, officers, employees, agents, representatives, successors, and/or assigns, and any other persons or entity associated with such event from and against any and all damages, injuries, losses, or expenses arising from, but not limited to, the participation in the Martial Arts Seminars, the action or omission of any of the above representatives, the use and/or publication of any pictures, photos or video reproduction, and the transportation of students to and from said Seminars. By acknowledging such waiver, I hereby state that myself/child is physically capable and medically cleared to participate in such an event. If such student is a minor (under age 18), the undersigned parent, in addition to acknowledging and executing the above waiver, agrees to indemnify Zodiac Combat Sports LLC, dba Nine Tigers Kung Fu, it’s owners, officers, employees, agents, representatives, successors, and/or assigns, and any other person or entity associated with such participation, for any damages sought by or on behalf of said minor.

Consent is given for using names and photos by Zodiac Combat Sports on its website & social media platforms & for any promotional/advertising materials for Zodiac Combat Sports; it is expressly understood that names and photographs will not be used for any other purpose without further written consent.
