I understand that I am required to attend the A5 Scholarship Banquet as a result of being a recipient of the Scholarship and could possibly forfeit my right if I am not in attendance. The banquet will be held in early August. Upon awarding of the scholarship, applicants will be notified of the exact date of the banquet. *

Application Criteria:
-Must reside in the Southeast Michigan/Northern Ohio area
-Attend a 2-4 year accredited college/university (If selected to receive the scholarship, proof of school registration must be presented)
-Submit a 300-word essay on the following: How will your degree help to positively impact your community? and Describe your need for assistance toward your college education.
-One letter of recommendation and one letter from someone who can identify your impact in the community.
