I, the customer/participant understand that the ropes course is a hazardous activity. I understand that the ropes course activity involves a risk of injury and even death and caution is required. Risks include but are not limited to; falling resulting in rope burns, abrasions, and pulled muscles. I hereby assume and accept any and all risks of injury or damage while participating at Wesleyan Woods Camp ropes course facility. Among other safety precautions I understand the need to climb only while wearing a climbing harness which is attached to a safety rope while being belayed by a trained belayer. I represent to Wesleyan Woods Camp that I have the skills and ability to safely participate in the ropes course activity.

In consideration of my being permitted by Wesleyan Woods Camp to participate in the ropes course and activities connected with the ropes course; I, for myself, my heirs, successors and personal representatives hereby release and discharge Wesleyan Woods Camp, its officers, board members, and employees from any and all claims, actions, suits, and proceedings, costs, expenses, injuries or damages to myself.
I agree to obey the rules for all activities including but not limited to; the use of guns, archery, paintball field, swimming, water slide, wet zip line, dry zip line, hayrides, etc.

Canoe & Boats; I agree to obey all of Wesleyan Woods Camp canoe rules. All persons in canoes must have a life jacket on, children under 16 must be accompanied by an adult. All life jackets and oars must be returned to the boat house or camp staff.

I agree for myself and heirs, guardians, and legal representatives, that in the event of any claims for personal injury, death, property damage, shall be prosecuted against Wesleyan Woods Camp, I shall indemnify and save harmless Wesleyan Woods Camp, its officers, board members, employees, and its affiliated organization from any and all such claims or causes of action by whom ever made and when ever presented.

As parent/guardian of the above child, I affirm that the information on this application is true and correct. Unless otherwise noted, my child is permitted to participate in all activities at Wesleyan Woods Camp. In case of necessary routine emergency treatment, I hereby grant permission to the Camps & Retreats Coordinator of Wesleyan Woods camp (or their appointee) to authorize such treatment by a qualified nurse, physician, or hospital, and that necessary information be released to the insurance company. Furthermore, I hereby authorize the release of all medical records (x-rays, test results) resulting from treatment to the Wesleyan Woods Camp. I also give permissions for my child to participate in and travel to any off-site activity sponsored by the camp. I further grant permission to the health officer to dispense Tylenol to my child while at camp.

I grant permission to Wesleyan Woods Camp and persons acting for or through them, the right to use, reproduce and/or distribute photographs, films, videotapes and sound recordings involving the participation of the individual identified on the form for promotional materials they may create.
