For Volunteer Licensed Dentists in California only:

Healthier Kids Foundation and the Santa Clara County Dental Society have partnered to provide oral health screenings for children at various dates throughout the year. We thank you for your interest in volunteering to help perform these screenings at local schools.

Please complete the short form below to be added to our list of available dentist volunteers. Additional instructions and orientation details will be emailed to you after this intake form is received.
