2025 Collegiate Wheelchair Program Grant Application
By completing and submitting this application, you are requesting grant funding from USTA Player Development's Collegiate Wheelchair Grant Program (the "Grant Program") for an existing or planned Collegiate Wheelchair Program to be used during this Grant Program cycle and agree to be bound by the terms and conditions set forth below and in any Grant award. 

If a grant is awarded, you will be expected to: 
          -  Communicate quarterly with USTA Player Development
          -  Provide semi-annual reports on the Program outlined in this application  
          -  Host one tournament, play day or other event, and  input the results into the Serve Tennis platform
          -  Plan to and represent your Program at the 2025 Collegiate Nationals currently scheduled to take place
             in Orlando
          -  Actively recruit  participants for the Program
          -  Follow safeguarding policies and protocols for those adults having regular contact or authority over                     minor athletes (see, e.g. the USTA Safe Play Conduct, Policies, and Guidelines 
             (https://www.usta.com/en/home/safe-play/usta-safe-play-conduct-policies-guidelines.html, and  
             always consult with your own advisors)
          -  Complete and submit a current form W-9 for the Program receiving the grant

The Grant Program cycle is from January through December.  The maximum amount available under the Grant Program is $5,000.00 (information supporting the requested dollar amount must be provided with the application) and you may request an amount up to and including $5,000.00.

USTA Player Development may also provide assistance with some of the Grant Program award requirements, including marketing and sponsorship ideas, and other materials. 

Due to the volume of applications received, incomplete or late applications will not be considered for the Grant Program cycle. 

The deadline to submit an application is 11:59 pm Eastern Time on Friday, January 31, 2025.
